Your organization's IRS Determination Letter
Your organization's EIN number
If your EIN/nonprofit is already in the system, you will not be able to use this inquiry form and need to log into your existing account.
Your organization's most current year of completed financials such as the Form 990, Form 990-EZ, etc.

We will review the information submitted to ensure your organization is a qualified 501(c)(3). If your organization is approved to participate, you will receive an email confirmation inviting you to login, connect your Stripe account and finish creating your profile!

Important Note: We reserve the right, in our sole discretion and for any reason, to decline to permit any organization, regardless of its current 501(c)(3) status, to participate.
